I declare that I have read Carlos Albizu University’s scholarship requirements, and I accept them without reservations or limitations.

I certify that all personal information provided in this application is correct, and that I meet this scholarship’s eligibility requirements. If this scholarship is awarded to me, I will abide by all present and future scholarship regulations and by any other applicable institutional requirements.

Similarly, once each institutionally funded course is completed, I will provide and official transcript showing the corresponding academic credits that justify and certify my performance in such course.

I certify that I request Carlos Albizu Institutional Scholarship strictly for academic reasons, and, if such scholarship is granted, I promise to use the funds solely for academic purposes.

I promise to participate in promotional and recruitment activities at Carlos Albizu University.

Note:

It is the responsibility of the Institutional Scholarship Committee to notify students about the outcome of their Carlos Albizu Institutional Scholarship application by email and postal letter. Student phone calls about their application status will not be accepted.
